HIPPA * Privacy

Each Zion Selah employee has a responsibility to clients and Zion Selah to uphold client privacy rights and maintain the security and integrity of client protected health information.

 

ACCORDINGLY, CLIENT PROTECTED HEALTH INFORMATION WILL BE TREATED AS CONFIDENTIAL, AND HELD, USED AND DISCLOSED ONLY IN COMPLIANCE WITH APPLICABLE LAWS/REGULATIONS AND AS SET FORTH WITHIN Zion Selah’s NOTICE OF PRIVACY PRACTICES (AND AS THE SAME MAY BE AMENDED AT ANY TIME AND FROM TIME TO TIME BY Zion Selah, AT ITS SOLE AND ABSOLUTE DISCRETION).

 

All employees will collect, use, disclose, maintain and store client protected health information in an honest, ethical, secure and confidential manner.

All employees must comply in all respects with the requirements of the Health Insurance Portability and Accountability Act ("HIPAA"), the Standards for Privacy and Security of Individually Identifiable Health Information promulgated at 45 CFR Parts 160 and 164, and all other applicable similar federal, state and/or local statutes, laws, ordinances, regulations, rules and interpretive guidance, and all amendments to any of the foregoing.

 

All Zion Selah employees will uphold and safeguard the rights of clients to the privacy of client protected health information by ensuring that client protected health information is used and disclosed only under the following conditions:

1.Each client or client's authorized personal representative has been provided with a copy of Zion Selah’s Notice of Privacy Practices and has signed a written confirmation of such receipt.

2.Each client or client's authorized personal representative has read, completed and signed Zion Selah’s Admission Service Agreement.

3.All Zion Selah employees must take all reasonable precautions to safeguard the confidentiality of client protected health information, including without limitation strict compliance with Zion Selah’s Notice of Privacy Practices (and as the same may be amended by Zion Selah, at its sole and absolute discretion, at any time and from time to time) and all federal, state and/or local statutes, laws, ordinances, regulations, rules and interpretive guidance relative to health information, including without HIPAA and all regulations promulgated thereunder, as well as any amendments to any of the foregoing.

The use and disclosure of client protected health information is permitted without specific authorization only when required for treatment, payment, and healthcare operations as set forth in Zion Selah’s Notice of Privacy Practices and/or applicable laws/regulations.

Non-compliance with this policy and associated procedures is a serious matter and may result in civil and criminal actions to the employee, in addition to disciplinary action up to and including immediate termination of employment.
